Key Responsibilities Urban Baby Beginnings is seeking a FT Maternal Child Home Visitor I (CHW-I) to work with our program.
A CHW-I is a lay health worker who will work with at-risk pregnant and parenting families and their babies, to promote healthy pregnancies, breastfeeding, and parent-infant attachment.
The goal is to improve birth outcomes, pair families with needed services, and reduce poor outcomes related to the lack of care coordination.
The CHW-I is a FT member of our UBB team, working collaboratively with doulas and other staff to manage a caseload of pregnant and parenting families.
Specific Roles:
The CHW-I will provide culturally sensitive support and serve as a role model in general, also will be a member of the team and work in a context of supervision and support.
Maintaining a minimum of 85% completed home visits for active clients, establishing a trusting relationship with at-risk families by initiating and maintaining regular client contact.
Providing 1:
1 support to clients through intensive home visitation, group facilitation and enhanced case management.
Initiating contact with families as early in pregnancy as possible, managing a caseload of moms and babies.
Seeking to establish a trusting relationship with the expectant parent, provide comprehensive education.
Completing weekly home visits with pregnant and parenting families in their homes and other locations such as doctor's office or other locations.
Providing check ins via phone call and email.
Serving as a co-leader in groups and classes.
Providing evidence-based information and resources to expectant, postpartum families and their children who have been heavily impacted by COVID19.
Assisting with birth planning and lactation support.
Conducting neighborhood canvassing.
Providing individual and group education related to prenatal, postpartum, and interconception health.
Conducting street outreach and outreach in public settings.
Providing education and materials to key influencers, such as barbers/hair stylists, coaches, fraternity members, pastors, neighborhood leaders, front-line workers, etc.
Conducting phone-based outreach.
Conducting presentations to consumer and resident groups in person and via social media.
Providing information on immunizations to individuals visiting health and social service organizations and facilities.
Advising on and developing visuals for collateral materials on all social media platforms.
Checking in on individuals to provide encouragement and social support.
Assisting with data collection and documentation of outreach activities.
Providing health education on preconception health, chronic disease self-management& holistic wellness Building health literacy and community trust in health systems Supporting individuals with management of longer-term side effects of COVID-19 illness.
Connecting individuals to services to address social and health needs through the Unite-Us platform and following up with individuals at various timepoints to verify linkage to services.
Performing other related tasks as needed or assigned.
Qualifications:
Graduation from high school or a G.
E.
D.
issued by state department of education required; 2 years of home visiting experience required; health or human services preferred but not required.
A related two-year (Associate) degree may be substituted for the health or human services experience.
Ability to communicate fluently in English and Spanish and work with multicultural families is a plus.
Prior experience in a CHW/Home Visitor or similar role preferred.
Previous course training as a CHW/Home Visitor required.
Excellent interpersonal skills required and ability to interact professionally and gain trust with culturally diverse individuals.
Familiarity with regional and local landscape.
Prior experience working with community resources and social services in focus region.
Ability to show empathy.
Ability to handle confidential information with discretion and professionalism.
Computer skills and proficiency in the use of handheld tablets.
Basic CHW/Home Visiting training will be provided to all hires to fulfill their roles.
Successful participation in CHW/Home Visiting training is a condition of certification.
Experience in developing trusting relationships with clients and colleagues Ability to work independently and within a team-based environment Desire to work with a diverse population of prenatal and postpartum clients Knowledge of working with expectant teens is desirable Ability to satisfy both inner-city/rural travel up to 30 miles from office or home location Must have a current and valid Driver's License or state ID Insured vehicle available for business use is required to conduct home visits Knowledge of women's health during the childbearing years Willingness to travel throughout Central Virginia and surrounding counties to a client is required Clean background check, VA Child Abuse Registry clearance, and driving record is a necessity Strong oral and written communication skills Computer literacy is required UBB provides equal employment opportunities (EEO) to all employees and applicants for employment without regard torace, color, religion, sex, national origin, age, disability or genetics.
